I use a French keyboard so I don't have a "\" below Backspace (just a large Enter key). If I try binding another key to +mlook in the config or the console (bind y +mlook) it accepts the command but does nothing. However if I misspell mlook (bind y +mlookx) pressing the key shows an error message that mlookx is not known so the key is being recognized. If I create a -mlook bind (bind u -mlook) that works to turn off an +mlook command at the console but then stays off (needs freelook=0 for this test). It really looks as if the "\" is coded to the command?
I thought I could get around this by assigning the scroll wheel to lookup & lookdown in the options, controls but that does not work at all.

Just turn on free look (freelook 1). You don't need to mess with +mlook, it's an artifact from Q1.

Ah! That explains a lot.
Abandoning mlook I added the following to my autoexec.cfg and it works great.
freelook 1 bind MOUSE3 "toggle freelook"
